Below are the mileage forms for 2010 - 2011.

Please note that these rates may be subject to change by the end of the season.

2 submissions are required.

First Half - Sept 1- Dec 31, 2010 - Due by January 5th, 2011

Second Half - Jan 1 - March 15, 2011 - Due by March 15th, 2011

The new Mileage form is in an excel format. If possible, please email the forms by the deadlines above. If that is not possible please fax the forms to me at (403) 382-3445.

Any late sheets will be subject to fine and/or loss of tournament mileage. There is going to be a very short time from submission of these forms until pay night and any late forms will be difficult to process. Thanks for your understanding.
